Milligrams measure weight. Millilitres measure volume. You can’t convert one to the other without knowing one more thing: how heavy the liquid is. Get that wrong and, for medicine, the mistake can be dangerous.

Why mg and mL aren’t the same thing
A milligram is a thousandth of a gram — a measure of mass. A millilitre is a thousandth of a litre — a measure of volume. Converting between them is like asking “how many kilograms is a metre.” You need a bridge, and the bridge is density: how much mass fits in a given volume.
People assume 1 mg = 1 mL because it’s roughly true for water, and it isn’t even true there. Water’s density is 1 gram per millilitre, which is 1000 milligrams per millilitre. So a thousand milligrams of water make one millilitre. Not one to one — a thousand to one.

That’s the whole thing. If you only ever remember one line, remember this: millilitres equal milligrams divided by how many milligrams sit in each millilitre.
Worked example, step by step
Say you have 2500 mg of olive oil and you want millilitres.
- Olive oil’s density is 0.918 g/mL. Multiply by 1000 to get mg/mL: 918 mg/mL.
- Divide your mass by that: 2500 ÷ 918 = 2.72 mL.
Try the same 2500 mg as water and you get 2.5 mL. Oil is lighter than water, so the same mass takes up more room. The density is doing real work here, not decoration.

Liquid medicine is different — read this
Here’s the thing that trips people up. If a bottle says 250 mg/5 mL, that means 250 milligrams of the drug in every 5 millilitres of liquid. Per millilitre, that’s 50 mg/mL. So a 250 mg dose is 5 mL — nowhere near the “0.25 mL” you’d get by treating it like water.
To do it right: read the concentration off the label, pick Liquid medication in the calculator, type that mg/mL figure, and it converts correctly. And for anything you actually swallow or inject, confirm the dose against the label or with a pharmacist. A converter is a helper, not a prescription.
The short version
mL = mg divided by density in mg/mL. Water is 1000 mg/mL, so a thousand milligrams of water is one millilitre. Other liquids need their own density, and medicine needs the concentration off its label — never a density, and never a guess.
